Conventional In Vitro Fertilisation (IVF)
The method of conventional fertilisation is selected when all sperm parameters are normal. It is essential for the success of this method to ensure a sufficient number of moving spermatozoa after the sperm treatment. Then the oocytes and processed sperm are placed together in a special cell-culture dish for a set time. During this time, there is a chance for the sperm to reach the oocytes on their own and fertilise them. That is why conventional IVF is widely considered as the most “natural” one.
- male infertility (oligospermia, asthenospermia, teratozoospermia, azoospermia)
- low oocyte quality,
- very low number of oocytes
Furthermore, ICSI is indicated for cases where a low contact of oocyte and sperm has been proven. With ICSI, spermatozoa are selected from their morphology and motility, and are transferred one by one into the cytoplasm of the oocyte, using a very fine glass pipette. The objective of the test is to ensure the entry of the sperm into the oocyte. By the ICSI method, very high fertilisation rates are achieved.
Micro-injection has been applied systematically and with great success since 1992. Furthermore, ICSI is appropriate for cases with low fertilisation, proven in previous attempts when conventional IVF has been applied.
In the ICSI procedure, an individual sperm is selected and injected into an oocyte. Until now, the only technique available to embryologists to select the sperm has been visual observation. Using PICSI procedure we are able to determine sperm selection in much the same way it happens in human biology.
Sperms are placed in PICSI dish containing samples of hyaluronan hydrogel. Hyaluronan is a naturally occurring biopolymer found in all human cells, including the gel layer surrounding the oocyte.
Mature, biochemically competent sperm bind to the hyaluronan where they can be isolated by the embryologist and used for ICSI. This procedure mimics a key step in the natural fertilization process, the binding of mature sperm to the oocyte complex. As a result, the selected sperm is essentially the same as one that would be successful in the natural reproductive process.
The research proved that hyaluronan-bound PICSI-selected sperm are, in the vast majority of cases, more mature, exhibit less DNA damage, and have fewer chromosomal aneuploidies.
Things to bare in mind
Due to the higher success rates of ICSI since Embryologists are able to select the best possible sperm for fertilization, most clinics abroad, particularly in destination countries such as Greece, Czech Republic and Spain, would only perform ICSI and not Conventional IVF. These clinics include ICSI in their prices and do not charge it extra.
